Our office will communicate with you beforehand to ask for some screening questions. You’ll be asked those same questions again when you arrive.
  2. NEW PATIENTS: Please fill out the medical and dental history forms online. If you can not, please come in 30 minutes before your appointment to fill out paperwork in your car. Paperwork is located on the first floor next to the elevator and below the hand sanitizer. Unless a minor, only patient will come inside.
  3. NEW PATIENTS: Please, Email the following items: Medical & dental history forms, Driver's License, Dental Insurance Card (front & back), Medical Insurance Card (front & back), Prescription Insurance Card (front & back). Email all items One day prior to your appointment.

When you arrive please stay in your car, text us when you are here, and we will text you when we are ready. We want to ensure the previous patient has exited and the workplace is fully sanitized before you enter.
  5. Patients / Guardians, please limit extra companions to only essential people in order to reduce the number of people in the reception area. If you prefer to accompany your child into the office, we will be limited to one adult at this time, please wear a mask.
  6. Appointments will be managed to allow for social distancing between patients. That might mean that you’re offered fewer options for scheduling your appointment.
  7. The waiting area is closed. You may see that our waiting room will no longer offer magazines, children’s toys, and so forth.
  8. We are making our entry and exit hands-free with sanitary arm or foot door openers.
  9. We are installing counter shields and barriers in our front office area.
